On the way to a more effective treatment for colorectal cancer

The drug candidate arfolitixorin is designed to improve the efficacy of existing chemotherapy rather than replace it, creating a clear pathway into clinical practice.

Isofol Medical is a clinical-stage, publicly listed biotechnology company developing arfolitixorin, a drug candidate designed to enhance the efficacy of established cancer treatments. The company focuses on metastatic colorectal cancer, one of the world’s most common cancers and an area with a substantial need for more effective treatments. In colorectal cancer, in the US alone, arfolitixorin has blockbuster potential – an estimated annual sales potential of more than USD 1 billion – and, over time, there is an opportunity to expand arfolitixorin into additional cancer areas and other indications.

The goal is for arfolitixorin to become a new component of first-line standard treatment for solid tumors, with an initial focus on metastatic colorectal cancer. The drug candidate is designed to improve the efficacy of existing chemotherapy rather than replace it, creating a clear pathway into clinical practice.

“Arfolitixorin is intended to enhance the efficacy of chemotherapy, which forms the basis of the first-line standard treatment used globally for several types of solid tumors. Isofol is therefore positioning the drug candidate as a potential complement – rather than a competitor – to established treatment regimens,” says Petter Segelman Lindqvist, CEO of Isofol.

First-line standard treatment relies primarily on the chemotherapy 5-FU in combination with folinic acid, where leucovorin is the established folate-based drug today. Folate-based drugs enhance the effect of chemotherapy on tumor cells while protecting healthy cells. The combination of 5-FU and folate is administered to more than 90 percent of patients diagnosed with metastatic colorectal cancer. This is where arfolitixorin can make a difference: with a more potent folate component, the efficacy of 5-FU-based treatment can be further enhanced. Arfolitixorin therefore has the potential to make a difference for a large and broad patient population.

Potential to compare with the competitor already in phase II

The clinical development plan is clear: if arfolitixorin can demonstrate superior efficacy to current treatment options without compromising safety or complicating treatment, there is a potential to create significant value. Isofol is currently conducting a phase Ib/II clinical study and is in the final stage of the dose-escalation phase Ib part, where increasing doses of arfolitixorin are being evaluated to identify the optimal doses for the next stage. The phase II study enables a direct comparison with leucovorin and is expected to begin before the turn of the year.

“The initiation and execution of the phase II study is an important milestone for Isofol. This is where we shift from a focus on safety and dose escalation to generating data that can strengthen the case for arfolitixorin’s clinical and commercial potential. Ahead of the study start, reinforced by what we have seen in phase Ib so far, we have increased the number of study sites and broadened the inclusion criteria to expand and accelerate patient recruitment,” says Petter Segelman Lindqvist.

The colorectal cancer market

Colorectal cancer is one of the world’s most common cancers. The World Health Organization (WHO) estimated approximately 1.9 million new cases and more than 900,000 deaths globally in 2022, and the disease is the second most common cause of cancer-related death. Metastatic colorectal cancer means that the disease has spread and that curative treatment is generally no longer possible. At this stage, systemic treatment – often chemotherapy in combination with targeted therapies – is the main approach to extending survival and maintaining quality of life. By enhancing the efficacy of chemotherapy, Isofol hopes to improve the outlook for these patients.

The global market for the treatment of metastatic colorectal cancer is estimated at approximately SEK 80 billion in 2032. A market analysis conducted by the consulting firm Back Bay Life Science Advisors highlights a substantial medical need and strong willingness to pay for improved first-line treatment options in metastatic colorectal cancer. Further, it confirms Isofol’s earlier assessment that arfolitixorin has the potential to achieve blockbuster-level gross sales – defined as more than USD 1 billion – in metastatic colorectal cancer in the US alone.

Colorectal cancer is increasing among younger people

One factor that could further strengthen the market outlook is the growing number of younger patients diagnosed with advanced-stage colorectal cancer, a disease long considered to primarily affect older people. In 2026, the American Cancer Society presented new data showing a marked increase in both incidence and mortality from colorectal cancer among people aged 20–49. Already in 2023, colorectal cancer had become the most common cause of cancer-related death among people under 50 in the US. Mortality in this age group has increased by just over 1 percent per year since 2005, and three out of four patients are now diagnosed at an advanced stage of disease. Likely contributing risk factors include higher rates of obesity, low physical activity, and dietary patterns with a high proportion of ultra-processed foods.

Several value-creating milestones ahead

The company is at a stage where several important value drivers are approaching: completion of the phase Ib dose escalation, the planned initiation of an multinational phase II study around the turn of the year 2026/2027, and ongoing research to analyze the potential for expansion into additional indications.

Last year, Isofol completed a successful and oversubscribed rights issue, providing the company with funding for the majority of the upcoming phase II study. During 16–30 November 2026, the subscription period for Isofol’s series TO2 warrants is open, representing an important opportunity for the company to further strengthen its financial flexibility. Holders of series TO2 warrants are entitled to subscribe for one new share per warrant, and the capital raised can contribute to financing the continued development of arfolitixorin.
